The Elsie Owens Health Center in Coram has recently become Hudson River HealthCare’s (HRHCare) 17th Health Center to be recognized as a Level 3 Patient-Centered Medical Home by the National Committee for Quality Assurance.
The patient-centered medical home model is intended to provide patients with personalized care and a long-term relationship with their physician. In achieving this status, the Elsie Owens Health Center displays the highest level of compliance with NCQA guidelines and exceeds excellence in providing patient-centered care.
